Additional Important Information:

Regarding the application and requested accompanying documents, the legal company name in its entirety should be reflected as it was submitted to the Internal Revenue Service (IRS) to obtain a Federal Employer Identification Number (FEIN) and the Kansas Secretary of State (KSOS) to enable the company to do business in Kansas. This full name must be listed on all documents. Examples:

  • Sole Proprietor: John Doe dba Doe Roofing & Construction
  • A Sole Proprietor is not required to file with the IRS for a FEIN or with the KSOS to do business. Sole proprietors should list their business name on the application as First Name Last Name dba Business Name as indicated in the above example.
  • Limited Liability Company:JR Roofing, LLC
  • Limited Liability Partnership:JR Roofing L.L.P.
  • A General Partnership is not required to file with the IRS for a FEIN.A General Partnership may file with the Kansas Secretary of State to do business, it is their choice.
  • Corporation: JR Roofing Inc.
